Directions

From the north shore: Take Northern State Parkway or Long Island Expressway (495) to Seaford Oyster Bay Expressway (135) south. Take 135 to exit 10, Old Country Road, and turn right at light heading west. Proceed west on Old Country Road for 2 traffic lights an then you will see a red tile roof medical center on right, pull into next parking lot.

From the south shore: Take Southern State Parkway or Sunrise Highway (27) to Seaford Oyster Bay Expressway (135) north. Take 135 to exit 10, Old Country Road, and turn left at light heading west. Proceed west on Old Country Road for 2 traffic lights an then you will see a red tile roof medical center on right, pull into next parking lot.
